What is The Chronic Disease Self Management Program (CDSMP)?

The Chronic Disease Self Management Program (CDSMP) is a community course which people with different chronic diseases attend together.

The small group courses are 6 weeks long, meeting once a week for 2 hours 30 minutes, and are led by at least one peer leader with a chronic condition. It is the process in which the program is taught that makes it effective The sessions are highly interactive, focusing on building skills, sharing experiences and support.

What is The Chronic Pain Self Management Program (CPSMP)?

The Chronic Pain Self Management Program (CPSMP) is a community course which people with chronic pain attend together. The CPSMP was developed for people who have a primary or secondary diagnosis of chronic pain. Pain is defined as being chronic or long term when it lasts for longer than 3 to 6 months, or beyond the normal healing time of an injury.

What is The Diabetes Self Management Program (DSMP)?

The Diabetes Self Management Program (DSMP) is a community course for people with Type 2 Diabetes.

The small group courses are 6 weeks long, meeting once a week for 2 hours 30 minutes, and are led by at least one peer leader with diabetes. The sessions are highly interactive, focusing on building skills, sharing experiences and support. The course teaches the life skills needed in the day-to-day management of chronic pain and to embrace life's opportunities.
