Working Together Conference Summary

The conference was a milestone in the development of chronic condition self management through the participation of internationally recognised academics and practitioners at the forefront of this discipline. Many states have supported the use of both the Flinders and Stanford programs as an approach to chronic condition management. This was an opportunity to see these complementary programs working together. The afternoon presentations focused on Men's Health in action.

The conference offered service providers, researchers and consumer representatives the opportunity to hear the latest research and share their perspectives on this important approach to disease management.

The conference  facilitated the sharing of information and will provide opportunities for participants to gain practical information and skills to further their implementation of chronic condition management in their workplace.
